New York State DOH Establishes Online Portal for Medicaid Provisional Temporary Provider Enrollment for the COVID-19 Crisis

There is a “Help functionality built into the portal that can assist along the way. The portal will ensure that no pertinent information is missing.
The application can be completed on a mobile device or tablet and is available 24-hours a day.
Provisional Temporary Provider Enrollment is available only for the following categories of service:
- Nurse LPN – 0521
- Nurse RN – 0522
- Nurse Practitioner – 0469
- Physician – 0460
- Physician Assistant (Registered) – 0462
